Adding more volume to the face, and by that, I mean more shadows and highlights. Right now, it looks very flat.
The eyes. Now, they're wonderful on their own, don't get me wrong, but I think if you're going for a more realistic look, the eyes would be smaller and closer together; eyes aren't at the very tip of the face!
And if you're going for the look without the hair, I would definitely clean up the edges because it gives it a more purposeful look rather than unfinished and rough.
I love the lips, though, and overall, the face (esp. nose area) has been beautifully painted
